Supportive Outreach Services (S.O.S.)
Supportive Outreach Services (S.O.S.) is a proactive and accessible partnership of community care providers. Together, social workers and health care providers offer bi-weekly clinics for vulnerable people suffering from mental health and substance use issues. Clinics are offered in downtown Owen Sound and in Hanover.
In addition, a mobile team can provide individual follow-up on non-clinic days.
Goals & Objectives:
S.O.S. supports the provision of wraparound health and social support to underserved community members, with a focus on those who are using substances and experiencing homelessness. Team members provide episodic medical interventions, mental health and addictions counselling, health teaching focused on harm reduction practices, support with navigating the health and social service system, access to basic needs in the form of food, clothes, hygiene products, facilitating referrals to ongoing support, and time limited case management support.
