Apr 2, 2024
This person-centered program works with people to determine what is personally important for them to achieve. Team members help individuals to identify meaningful goals and effective ways for each person to reach those goals. We work alongside you, enabling you to exercise your rights and to enjoy a meaningful life in your community while striving to achieve your full potential. We work to promote health, housing, purpose, and community. The program uses a holistic, strengths-based approach to support individuals towards living a full, independent, and satisfying life.
