Intensive Case Management

Intensive Case Management is a program for individuals with serious mental health challenges who are looking for coordination, advocacy, and life skill development support.  This program can assist individuals in getting connected to new and appropriate resources in their communities and develop skills that will assist them in living a fulfilling and meaningful life.  Case Managers meet with individuals in their home or community and help them identify and work towards personal recovery goals that will improve their quality of life. 

Goals & Objectives:  

• Support individuals in meeting their basic needs (e.g., food, shelter, finances) and encourage the development of skills needed to live independently and improve quality of life. 
• Promote independence and provide ongoing support as needed to attain recovery goals. 
• Coordinate required services across the mental health system, and other service systems.  

Is This Program for Me? 

• You are 16 years of age or older living with serious mental health and/or addiction challenges 
• You reside in Markdale, Southampton, Kincardine or the surrounding areas
• You have a willingness and a commitment to engage with your worker  

What We Offer:  

• Support to access counselling, and/or psychiatric consultation 
• Education and support in life skills development 
• Connection to other appropriate support services 
• Information, liaison, advocacy, consultation, and collaboration 
• Connection with Brightshores Health System working in collaboration with the Community Treatment Order (CTO) Team 

How Do I Get Connected? 

• Referrals can be made by any of the following: Hospital inpatient units; family doctor; as well as community agencies and health services throughout Grey/Bruce. 
• Call our Central Referral and Intake Team at 519-371-3642 ext. 3212. This team is available 8:30 a.m. to 4:30 p.m., Monday to Friday.
