Primary Care Telemedicine Program
The Primary Care Telemedicine Program provides primary care for individuals who either lack primary care or require more comprehensive primary care than they are currently able to receive from their family physician or nurse practitioner. Program services are available to individuals with a serious mental illness and/or addiction issue for which they are receiving local mental health and/or addiction services.
The Nurse Practitioner and Registered Nurse can provide a variety of services, including, but not limited to: comprehensive health assessment; monitoring and management of chronic diseases such as diabetes, arthritis, asthma and heart disease; prescribing and monitoring of medication; immunizations; health teaching; referral to specialists and other human services; and consultation and service coordination.
The program is delivered in collaboration with the Brockton and Area Family Health Team and four physician groups. The program is based in Walkerton and Owen Sound with outreach to Durham, Hanover, Chesley, Paisley, Kincardine, Southampton, Wiarton and Port Elgin.
A Drop-In Clinic is also offered Tuesday afternoons from 1:00 p.m. to 3:30 p.m. in the Telemedicine Office in Suite 205 at 1101 2nd Ave. East, Owen Sound.
Speak to your mental health and/or addiction worker about the program, and ask them to complete a Primary Care Telemedicine referral form with you. Once your referral has been received, you will be contacted by a member of the program. For more information, please contact the office below:
